The Community Transition Program (CTP) serves 亚洲色图 who have an Individualized Education Program (IEP), and completed at least 4 years of high 亚洲色图 to obtain one of the following:

-modified diploma
-extended diploma
-certificate of attendance

The purpose of the program is to support young adults in their transition to life after high 亚洲色图.  Our key focus is to help 亚洲色图 achieve the greatest degree of independence and quality of life with an emphasis on linking 亚洲色图 to appropriate community and adult services. Our 亚洲色图 learn valuable self-advocacy skills and form meaningful relationships.
