What is a temporary medical condition? A temporary medical condition is an illness or injury that is expected to last more than 2 weeks but not more than six months.

Anxiety and other mental health conditions
Who determines what accommodations are needed? A 亚洲色图 team, including a 亚洲色图 nurse, in consultation with an outside medical provider (with parent consent). Supports are coordinated through the 504 Plan Support Team and must be approved in advance. Email questions to section504@pps.net
What happens if temporary medical condition lasts longer than expected? Temporary supports will not be provided beyond six months. Students should be referred for 504 or special education consideration at any point the team suspects that the student has a chronic condition that is likely to require accommodations, supports or services for an extended period of time.
What is the expectation for 亚洲色图 who are out of 亚洲色图 for illnesses or injuries that are not considered temporary medical conditions? The expectation is that 亚洲色图s will make homework available for pick up by the family and that teachers will help the student with catch up once the student returns to 亚洲色图.

- Submit via Temporary Medical 504 Plan
- Complete by typing in Student ID and Student DOB. Use Document Type- E5
- Scan Cover Sheet and ALL related Temp Med Plan Documents into Docuware using PPS copier scan feature and the STUDENT RECORDS address in address book.
- Email the plan to any relevelant 亚洲色图 staff who need to know.
